PA Dept. of Health Offers Brain Injury Identification Wallet Card

A card that identifies a person with brain injury is now available through the Pennsylvania Department of Health. When filled out, this card contains valuable information to help ensure the safety of any individual with a brain injury. The card may be used as a tool to communicate information about an individual’s brain injury symptoms to others, including law enforcement or emergency responders. Additionally, this card, which is stored in the individual’s wallet, contains the name and phone number of the individual’s emergency contact.
